Expert Evaluation and Management of Chronic and Complex Medical Diseases
Transitional Care after Hospital or Nursing Home Discharge
Expert Physician Coordination of Home Care
Detailed and Complete History and Physical Examination
Resting Electrocardiogram
(A recorded tracing of the Heart’s electrical activity)
Spirometry and Pulse Oximetry
(Measurement of Lung Capacities and Respiratory Status)
Anticoagulant Management at Point of Care
(Adjustment of Coumadin or Warfarin treatment at time of visit, not by telephone follow-up)
Necessary Laboratory Tests with sampling performed at Home
Some Radiology Services Performed at Home
We will coordinate patient care with your current Physician.
